Mr. Oda, a member of our GSSG, serializes a column for “ITmedia” titled “Causes and Countermeasures for quality defects that lead to product recalls.”

Serious product accidents are on the rise, and many of their causes are classified as “product-related.”  However, systems for building and verifying quality are in place at the design and mass production sites.  Despite this, why do serious accidents and recalls still occur?  This article will organize the structure of design quality and mass production quality and systematically examine the mechanisms by which quality defects arise.
